Memorial- Youth in Transition Program

Healthy Youth in Transitions provides one-to-one life coaching services to youth in foster care, relative care, DJJ, and LGBTQ youth. We provide intensive case management as well as home visits, groups, educational assistance, and job readiness as well as individual and group therapy.

Eligibility Requirements: 

   -  Must reside in Broward County

   -  Be between the ages of 15-21

    - Be in foster care, adopted from foster care, or relative care

    - DJJ or LGBTQ youth
